Certified Nurse Assistant

One of the most critical members of the Hospice El Paso team is the Certified Nursing Assistant (CNA). Under the supervision of the Hospice nurse, the CNA provides personal care services according to each patient’s individual plan of care. In addition, the CNA can accurately record vital signs and will monitor and report significant changes in the patient.

The versatile Hospice CNA provides and educates personal care techniques to the patient, family, and caregiver. These may include assistance with bathing, skincare, grooming, oral care, bathroom needs, dressing activities, and other tasks falling under the CNA scope of practice. With the care, compassion, and dedication provided by the Hospice aide, it is easy to see how invaluable the CNA is to the Hospice El Paso team!

Chaplains

Chaplains provide spiritual support in various forms, including ministry of presence, spiritual counseling, prayer, and other duties according to the patient’s belief system. Chaplains listen with compassion to families during their grieving process and in times of crisis. Their role is to assist patients and families in utilizing their own faith and their own denominational ministers with the goal of empowering them to have a peaceful death and leave the family with hope. Services include:

Medical Director

The Hospice Medical Director oversees each patient’s individualized plan of care. In addition, the Medical Director provides the necessary support to the hospice team through attendance at weekly and, as needed, interdisciplinary team conferences, ensuring an appropriate plan of care. Furthermore, the Medical Director collaborates with other physicians to ensure availability for every patient when updates to orders and medications are needed.

Registered Nurse

The Hospice Registered Nurse Case Manager is a professional who holds the responsibility of collaborating and coordinating the plan of care through all necessary disciplines, including but not limited to the Medical Director, Attending Physician, Social Worker, Chaplain, Bereavement Coordinator, and/or Certified Nursing Aide.

The primary purpose of the Registered Nurse Case Manager is to serve as an educator for the patients and their families; providing guidance on end-of-life care; ensuring the families have a true understanding of how to care for their loved one; and explaining what to expect as the disease progresses, while ensuring comfort for the patient. Furthermore, the RN Case Manager is able to assess the medical needs of the patient and communicate with the physician to obtain necessary orders to ensure patients’ needs are met.

Social Workers

Social workers are strong advocates for patient’s right to self-determination during a life-limiting illness. Social Workers use their expertise in working with populations from varying cultural backgrounds, socioeconomic status, age, gender, and denominations. Social workers ensure patients have access to resources to meet their psychosocial needs at the end of life. Services include:

  • Conduct a thorough psychosocial assessment upon admission.
  • Make visits based on the established plan of care
  • Link to community agencies for needed resources
  • Guide families with funeral arrangements
  • Facilitate advanced directives
  • Assist families in navigating government programs
  • Provide for family members via humanitarian parole letters, excuse of absence letters, and FMLA documentation.
  • Educate patients, families, and the community on hospice services and philosophy.
  • Collaborate with other agencies to provide low-cost or free services for uninsured patients.
  • Continually assess caregiving needs
  • Coordinate arrangements for travelling patients
  • Engage patient and family in crisis intervention
  • Continually assess abuse, neglect, and exploitation and make referrals as appropriate.
  • Mediate between patients, hospice staff, physicians, hands-on caregivers, and other agencies as appropriate.
  • Attend to professional roles and adhere to the NASW Code of Ethics
  • Act as a broker by knowing community resources and eligibility and providing follow-up
  • Provide support through the grieving process, offering counseling and connecting families with bereavement services.
